During your first visit to Berry Farms Dental, you will meet with one of our dentists and our caring team. You will also be welcome to take a tour of the office. Dr. Johnson or Dr. McVey will complete a thorough evaluation of your teeth and gums. Time will also be spent discussing your smile concerns, needs, and desires in order to give the most personalized care possible. After the evaluation, our team will work with you to create a treatment plan that best fits you. A staff member will also be happy to explain your insurance benefits and file your insurance.
What to Bring to your Dentist Appointment
We know that your time is valuable. In order to save time, please fill out your paperwork via our website using the new patient online forms.
Additionally, please bring the following items to your appointment:
– Driver’s License
– Dental Insurance Card
*If you are unable to complete the online forms prior to your appointment time, please arrive 20 minutes prior to your appointment to complete patient registration forms.
*A parent or legal guardian must be present for children under the age of 18.
